We collect Personally Identifiable Information
Personally Identifiable Information is information that identifies you or can be used to identify or contact you. Such Personally Identifiable Information may include your name, address, email address, telephone number, birth date, and billing and payment information. We request this information as part of our registration process. Without providing such information, you will not be able to participate in the Sensable Health program.
Information from Other Sources
We may display personal testimonials, including your first name, of satisfied participants of Sensable Health on our site. This will ONLY occur with your consent. If you wish to delete your testimonial from our Website, you can contact us at email@example.com.
Your Mobile Device
Healthy LaunchPAD will collect information about your usage of the application (such as the dates you last synced with the Internet or the dates you last used certain features). When you sync your device with the Website, we store some of this information. We utilize this information to send you program messages.

Protected Health Information
As an essential part of the Sensable Health program, Healthy LaunchPAD will be collecting information that is Personal Health Information (PHI). This information will include information about your health conditions and laboratory data. This information is being collected for the purpose of better understanding the reasons associated with weight gain and related conditions and creating a specific wellness plan for you. It is specifically NOT BEING COLLECTED for the purpose of making any diagnosis, making any medical assessment, making recommendations about any medication, providing a treatment plan, or for any health care decisions or recommendations. Sensable Health is a wellness program and is NOT a health care program or a medical provider. Generally, we will only share PHI with someone with YOUR specific written request or approval. Any questions you may have regarding your PHI, laboratory data, medical condition, medications, medical symptoms you may be experiencing WILL NOT be addressed by Healthy LaunchPAD, and must be directed to your personal health care provider.
WHAT DO WE DO WITH THE INFORMATION WE COLLECT?
In general, we use the information collected on our Website to personalize your journey through Sensable Health, to provide you with timely information about your progress in the program and to send you messages that might be helpful. If you do not wish to receive these messages, you may cancel your account with Healthy LaunchPAD at any time.
We may also use the information gathered from the Sensable Health program to perform statistical analysis of various aspects of the program, to analyze and evaluate issues relating to nutrition, activity, stress and sleep, as well as participant activity in the program in order to improve the Sensable Health program for future participants. All such information we use for this purpose will be de-identified.
Laws and Legal Obligation
We may also disclose your information (including Personally Identifiable Information) if we believe in good faith that we are required to do so in order to comply with an applicable statute, regulation, rule or law, a subpoena, a search warrant, a court or regulatory order, or other valid legal process.
Release of Unidentified Information To Interested Third Parties
We may provide aggregate information about our program enrollees that does not allow you to be identified or contacted. For example, we might inform third parties regarding the number and demographic information of our participants or results from participation. The third parties to which we may provide this information may include potential partners, researchers and other parties.

Notwithstanding the above commitments to protect your information (including Personally Identifiable Information) from loss, misuse or alteration by third parties, you should be aware that there is always some risk involved in transmitting information over the Internet. There is also some risk that others could find a way to thwart our security systems. As a result, while we strive to protect your information, we cannot ensure or warrant the security and privacy of any information you transmit to us, and you do so at your own risk.
WHAT CHOICES DO YOU HAVE REGARDING THE COLLECTION, DISCLOSURE AND DISTRIBUTION OF PERSONALLY IDENTIFIABLE INFORMATION?
In order to participate in the Sensable Health program, you must give us permission to use your Personally Identifiable Information as described above. If you do not wish to share your information as described above, DO NOT access or use the Sensable Health APP or sign this form below or continue your enrollment in this program.
CAN YOU UPDATE OR CORRECT YOUR PERSONALLY IDENTIFIABLE INFORMATION?
You can at any time contact us in order to (1) update or correct your Personally Identifiable Information, or (2) drop out of the program and delete the Personally Identifiable Information maintained about you on our systems. Use the Support Team button in the App to do so.
We will, after receiving your request, make sure that all Personally Identifiable Information stored in databases will be updated, corrected, changed or deleted, as appropriate. We will respond to your request within 30 days.
